What if we stopped thinking in silos – primary care, hospital care, social and medico-social care – moved toward truly integrated healthcare? Care pathways are evolving into health pathways, and even life pathways. This approach supports better coordination and seamless communication across the system, ensuring that patients receive the right care, from the right professionals, in the right place, at the right time… and as close to home as possible.
